Sample is inconsistent when pressing pad (See video example...)

Hey guys,
I've been stumped on this issue for a couple days now.

When triggering a sample, sometimes the sound changes after every 2-3 trigger of that sound. Mostly noticeable on "Roll". To my knowledge all the modulation sources are turned off completely.
