LFO frequency: converting to and from actual Hz

The attached Mopho Reference Guide has, on p. 4, a graph for converting LFO frequency values to and from actual Hz. I received permission from the author, Mike Hall, to repost it on the Prophet 08 forum, but he no longer has the experimental data from which the graph was derived.

Has anyone else created a table for converting LFO frequency values to Hz? A lot of generic patch recipes express LFO frequency in Hz, including this one from Sean Costello, of Valhalla DSP:

At some point, I want to try a Prophet 08, for the following patch:

VCO1 = saw, with ~4.4 Hz LFO pitch mod
VCO2 = pulse, with ~5 Hz LFO pitch mod, and some other rate for PWM

This would get close to the Polymoog Keyboard Vox Humana (or "Vox Numana" as I like to call that sound).
